Child details changes for childcare providers

About this form

Childcare providers should complete this form when there has been a change for a child in your service.

Changes include children who:

• are no longer attending
• are starting ECE
• are reducing their hours
• will be absent for 3 weeks or more
• have a change in fee
• are charged or have changes to their home-based education top-up payments.

Please make sure you submit this form for only one family at a time.
