Please complete this form if a child in your care is about to turn 18 (you will need to complete a form for each child turning 18).
If you've already submitted a form, you don't need to complete another form. We will aim to update your record within 5 working days.
When a child turns 18, we usually stop including them in payments.
However, we may be able to continue payments if they:
- are in school or tertiary education and attending regularly, and
- remain financially dependent on you.
If you don't provide the information we have requested, when the child in your care turns 18,
your benefit will be automatically reassessed using automated decision-making,
unless you are receiving any of the payments listed below.

If you are getting any of the payments listed below, we may need to review your entitlements.
Please get in touch with us before the child in your care turns 18 and finishes education or your payments will stop.
If you are receiving any of these payments, you still need to complete this form.
